If you have changed your child’s name due to a court order or any adoption procedure, it is mandatory that you get the same name change reflected on his/her social security card in order to ensure a smoother future for your child. One of the main problems that may arise if you delay is that the process would be much more complex as the time passes and as your kids grow up, they may face difficulty with employment in the future along with a reduction in the social security benefits received due to an incorrect documentation of his/her wages to the record.

The Procedure

The process of filing for a name change for your child is the same as with an adult. The steps are simple enough, gather all the necessary documents, fill in the application form correctly and submit it at the nearest social security card office to ensure that the changes are correctly submitted. With the online application, you can even simplify the process further by mailing the form to the concerned website, upon which the name change process will be intimated.

The Required Documents

For changing name on social security card of your child, you would need to submit three major types of documents that would ensure a smooth processing. The first is a document confirming the citizenship status of your child. The next would be proof of the legal name change. For this purpose, you can choose the finalized adoption decree, the court order or even an amended birth certificate with the new name.

Finally, you would require an identity proof for your child, preferably one with a photograph, but other documents with sufficient information of the child’s name, age, birth date and/or parents’ names.

If you are looking to reduce the hassle while changing name on social security card of your child, these documents are mandatory for the application.
